Neck Pain After a Car Accident: What to Do Before Considering Hands-On Care

Quick Answer

After a car collision, do not assume neck pain is simply a muscle strain or book a neck adjustment before considering injury assessment. Whiplash describes a neck injury caused by sudden head movement; pain, stiffness or headache may appear hours later. Emergency symptoms, possible concussion and neurological changes need medical attention first. Once serious injury has been considered, gentle everyday movement and an individualized recovery plan are usually more useful than prolonged bed rest or a routine neck collar.

First, check whether this is an emergency

In Oakland, call 911 for a serious collision or concerning symptoms rather than driving yourself to an appointment. After a head or body jolt, the CDC lists a worsening headache that does not go away, repeated vomiting, seizure, unusual confusion or difficulty waking as concussion danger signs. If you have an injured neck with new weakness, tingling on either side, difficulty walking or sitting upright, an electric-shock sensation down the back or limbs, or severe pain despite usual pain relief, seek urgent medical evaluation. Do not drive or cycle if your symptoms make either unsafe. A clinician may need to assess for fracture, nerve injury or brain injury; a chiropractic visit is not a substitute for that assessment.

What to do after you have been medically cleared for a simple neck injury

  1. Record the sequence. Note when the collision happened, whether your head struck anything, when the pain began, any headache or dizziness, and whether symptoms travel into an arm. Symptoms can develop several hours after the event, so an initial lack of pain is not proof that nothing happened.
  2. Keep ordinary activity within tolerance. The NHS advises trying to continue everyday activities rather than resting the neck for long periods. Start with comfortable movements and ordinary tasks; reduce an activity if it sharply aggravates symptoms. Do not force a painful stretch or return to contact sport before medical advice.
  3. Discuss pain relief safely. The NHS mentions common over-the-counter medicines, but your pharmacist or medical clinician should check whether a product fits your other medicines, pregnancy status, stomach or kidney problems, or other conditions. Follow the label. Do not use pain relief to mask worsening neurological symptoms.
  4. Skip the routine collar. A neck brace or collar does not help typical whiplash recovery according to the NHS; use one only if a treating clinician specifically prescribes it for an identified injury.
  5. Set a review point. If symptoms have not improved after about a week, pain relief is not working, or you are worried, contact a primary-care clinician. Persistent pain may merit physical therapy or another assessment rather than repeating a treatment that is not helping.

Where hands-on care may fit—and where it does not

After appropriate medical assessment, a person with uncomplicated musculoskeletal neck discomfort may discuss activity advice, exercise, physical therapy and carefully selected manual care. Different hands-on techniques are not interchangeable: mobilization without a thrust differs from spinal manipulation. The NIH’s NCCIH says evidence for manipulation in acute neck pain is limited and should be interpreted cautiously. It also explains that neck manipulation has been linked to rare cervical artery tears that can lead to stroke; the causal relationship is debated, but informed consent matters. A recent neck injury is a reason to disclose the entire collision history and ask what technique is proposed, why, what the alternatives are, and when to stop or refer. There is no promise that manipulation will speed recovery.

Kinetic Chiropractic describes its chiropractic care, but Dr. Lisa Koenig’s Oakland office should not be your first stop for emergency symptoms. If you have already been medically evaluated and want to discuss nonurgent options, the new-patient information explains the consultation process. You do not need to become a patient to use these steps.
